Diabetes Program
An estimated 34.1 million adults have diabetes in the United States. It is well established that diabetes disproportionately affects people of lower socioeconomic status, and uninsured diabetics are more likely to experience diagnostic and treatment delays. At People’s Health Clinic we have a multidisciplinary Diabetes Program dedicated to the care of our patients with diabetes. Since our program began in October 2020, we have enrolled more than 170 patients who are learning how to successfully manage their diabetes. We also help patients access free or affordable diabetic medications and supplies. PHC has been recognized by the American Diabetes Association as meeting the national standards for Diabetes Self-management Education and Support.
Hypertension Program
One-third of American adults are affected by high blood pressure with only 1 in 4 adults being controlled. One of our top priorities at People’s Health Clinic (PHC) is to improve our diagnosis, management, and follow-up of hypertension in order to reduce the risk of heart disease, stroke and other co-morbidities. This past July, PHC implemented a hypertension program guided by the American Heart Association’s Target: BP initiative which recommends a hypertension control rate of over 70%. PHC welcomed over 50 donated home blood pressure monitors and has enrolled over 75 patients into our hypertension program.
