Eligibility and how to apply

We’re here to help you apply for healthcare at our clinic and determine if you’re eligible.


  1. Eligibility
    You must meet BOTH of the following requirements:
    – MUST NOT have health insurance
    – MUST be a resident of either Summit or Wasatch Counties in Utah
  2. Applying for service
    To apply for service and make an appointment, please call (435) 333-1850. You can only make an appointment by phone.
  3. Appointments
    Patients are seen by appointment ONLY.
  4. On-line service
    Tele-health appointments are available upon request by calling the clinic.

IMPORTANT:
If you are in a medical emergency please call 911 or visit your local Emergency Room.

Mobile Outreach Crisis Team (MCOT)

MCOT is a group of peer support specialists trained by Wasatch Behavioral Health to deescalate and support in situations where an individual is suicidal. For more information on MCOT contact Rick Killpack rkillpack@wasatch.org. The phone number for the PUBLIC when in crisis is: 800-273-8255

Early Childhood Alliance

The Early Childhood Alliance engages, educates, and supports members, children, parents, and the community-at-large to prioritize accessible, excellent and equitable outcomes for children in the Wasatch Back from the prenatal stage through age three. We are located in Park City, Utah.
